I am Dr Triveni Joshi, a Child & Adolescent Consultant Psychiatrist with over 17 years of experience working in psychiatry, and over 14 years experience working with children and their families.

I find it very rewarding to help children overcome mental health difficulties. The right support can substantially impact improving emotional well-being, school performance, family relationships, and social skills. I believe that parents have the best understanding of their child and what difficulties they are facing. My goal is to work as a team with parents, to provide them with the right knowledge and tools to support their child.

My approach is child or young person centred. I am very aware of how difficult it is for the child or young person to see a doctor, let alone a psychiatrist. I tailor my approach to support the child with the process.

I have extensive experience across a wide range of paediatric mental health conditions, including autism spectrum disorders (ASD), attention-deficit hyperactivity disorder (ADHD), anxiety disorders, obsessive-compulsive disorder (OCD), body dysmorphic disorder (BDD), depression, bipolar disorder, psychosis and behavioural difficulties. Children deserve the opportunity to grow up and experience their lives without unnecessary fear, which is my ultimate goal of helping a child through any mental health symptoms.

I love to work with children, and I wholeheartedly believe that early intervention is one of the best ways to manage long term mental health problems successfully.

My approach to Clinical Management

I believe in a person-centred approach. My approach to working with young people is to have unconditional positive regard for young people, seeing and respecting their view point, but also being honest and transparent in my interactions. I believe the first step is getting to know the young person and their unique context. I aim to create a trusting and supportive environment to show the child that their active participation in their treatment is essential and help them to do that in our sessions.

It is important to me to understand the difficulties from the child’s perspective. When assessing symptoms, I use a combination of informal and flexible interviews with standardised clinical assessments to evaluate their symptoms.

I utilise the biopsychosocial (biological, psychological, social) model for formulating the mental health symptoms. I take into account genetics and neurobiology and psychological aspects of personality and social and environmental factors to understand why the young person may be struggling.

During the diagnosis and assessment stages, I believe that involving the parents is essential. Parents know and understand their child better than anyone, so I also take the time to understand the problem from the parent’s perspective while respecting the young person’s views on their parent’s involvement.

Once the assessments are completed I will be providing a detailed and accurate diagnosis, informed by my clinical experience as a psychiatrist and the evidence base. I also believe in revisiting and reviewing diagnosis and treatment plans alongside young people and their parents at regular intervals to ensure we are moving in the right direction and everyone is feeling listened to at all times.

My approach to Treatment Plans

When it comes to creating a treatment plan, my extensive clinical experience enables me to combine the latest evidence-based methods while taking into account personal preferences to develop a treatment plan that is realistic and beneficial. Treatment plans typically involve a combination of psychoeducation, therapy and lifestyle changes, and medication if necessary.

Once the condition or area of difficulty has been identified, I provide psychoeducation (psychological education) about the condition and create a personalised treatment plan with strategies for both the young person and their parents. By this stage, the diagnosis itself can provide a huge relief. Increased understanding of any learning or social difficulties improves relationships and skills inside and outside the home. When providing information and resources, I always consider the child’s age and their capacity to understand their diagnosis.

My approach is holistic geared towards managing symptoms by taking into account lifestyle, nutrition, exercise, stress reduction techniques and sleeping habits. For example, mindfulness skills can help reduce stress if they are relevant and work well for the child.

Additionally, Nutritional manipulation can decrease autoimmune difficulties. Clinical research shows that often mental health conditions can be accompanied by immune problems or gastrointestinal dysfunction. Nutritional manipulation can decrease autoimmune difficulties and enhance optimal neurodevelopmental health, so I may recommend blood tests or refer to other specialists as appropriate.

My approach to medication

I believe that medication can help alleviate symptoms, but the decision to prescribe medication always depends on the young person’s preferences and how their parents feel about it. My psychopharmacology training means that I have the knowledge to help young people understand the effects of medication, weigh up the benefits and any risks, and support them in trialling medication and advising appropriately if they choose to go ahead.

My approach to therapy

My approach to therapy is flexible and dynamic. I have a strong psychotherapy background and have training in both Cognitive-Behavioural Therapy (CBT) and Dialectic-Behavioural Therapy (DBT). Rather than focusing on a specific approach, I combine techniques according to the individual and explain how using a particular method can help them. The skills gained in therapy help challenge and overcome negative thoughts, which help address the difficulties that the young person may be experiencing, improve confidence and increase self-esteem.

I come from a strong systemic background and believe strongly in supporting families during the therapeutic process. Psychoeducation, family work, addressing their questions, supporting them in accessing external support.

If appropriate and needed I will be supporting the parents through any processes for education related support.

Work Experience

I have worked across various settings in the NHS and in the Private Sector.

My roles have included clinical and leadership roles and spanned across community CAMHS clinics, Residential Schools for children with Autism and Learning Disability, looked after children in residential settings and Tier 4 inpatient settings at various levels of acuity.

In my leadership role, I have managed clinical teams and have been involved in developing various models of care and assessment and outcome tools to enable us to deliver the best care and outcomes for young people.

I worked as the Medical Director and Consultant Psychiatrist at Huntercombe Hospital Maidenhead and as the Group Clinical Director at Cambian group for their Children’s Services across the country. Before I joined the private sector, I worked in the NHS as a Consultant Psychiatrist and was involved in integration of the CAMHS/ CAMHS LD pathway in the trust.

Currently I am working in Cygnet Group as the Medical Director at Cygnet Joyce Parker Hospital.

I’m a General Medical Council (GMC) registered Child Psychiatrist, and a member of the Royal College of Psychiatrists and the British Pharmacological Society. In addition to my specialist training in Child and Adolescent Psychiatry, I hold diplomas in Cognitive-Behavioural Therapy (CBT), certificate in Dialectic-Behavioural Therapy (DBT) and ADOS Autism assessment.

My personal interests

I’m originally from Mysore in Southern India and grew up speaking Hindi, Telugu and Kannada fluently and English. I’m passionate about nutrition and fitness, so I’m keen to integrate this knowledge into my work as a psychiatrist. I believe that physical health and a healthy lifestyle and particular attention to nutrition can significantly improve mental health, which I often bring into my practice. I also have a keen interest in sustainable fashion and sustainable living.
